RECONSTRUCTION WITH FREE FLAP USING EXPANDED TISSUE




Abstract
Tissue expander became one of the most popular techniques in the field of plastic surgery since it was introduced in 1976 by Radovan.
Although numerous reports on both clinical and experimental experiences have been advocated, still there remain some problems in clinical application for promising good results.
We have experienced two cases of reconstruction with free flap using expanded skin flaps. The 1st case was a girl who had amputated above knee, and lost the soft tissue on lower abdomen, groin and medial aspect of left thigh. Then she could not take leg prosthesis because of the unstable scars on the above area. We transferred the latissimus dorsi and scapular free flaps by anastomosis of contralateral inferior epigastric vessels after expanding this flap.
The 2nd case was a girl who had unstable scars over the frontotemporal area. She was complaint of alopecia and depression over that site. We performed the expanded contralateral scalp free flap. The affected side scalp could not be expanded because of the non-unioned calvarial bone and pervious cranieplasty state under the affected site. The donor site was closed by -direct closure.
The postoperative results were satisfactory in functional and cosmetic aspects during followup.
